Dental Sliding Fee Program

Minnie Hamilton Health System can assist patients with costs of health care services by offering a Sliding Fee program. Based on income, a patient may only have to pay 50%, 60% or 75% on select in-house Dental services, which include examinations, restorations and extractions. It does not apply to crown and bridge services or certain cosmetic procedures, including 2 surface (or greater) posterior composite restorations, composite veneers, or vital bleaching.

An application can be obtained from the MHHS Dental patient registration window. Once the application is received and processed, you will receive a determination letter from the hospital. The Sliding Fee Program is based on federal poverty level guidelines.
